Supplemental Life

Your monthly premium for supplemental life is based on your age, tobacco use and how much coverage you elect. It can increase during the year if you get a raise or if your birthday pushes you into a higher age group. You can enroll, change or cancel coverage at any time. Evidence of Insurability (EOI) may be required if you enroll more than 30 days after the date you are first eligible or if you increase coverage.
