Resources for Living services, administered by Aetna, are confidential, completely voluntary and provided at no cost to you and your dependents. You can access Resources for Living for confidential counseling, as well as phone and online assistance for a variety of needs, including:

  • Stress management

  • Adoption
  • Child and adult care, including special needs situations
  • Parenting and child development

2018 Well-Being Incentive Program and Fitness Reimbursement

The following program is effective Jan. 1, 2018. Further information will be provided in early 2018. Review the FAQs in the Resources box to learn more.

Well-Being Incentive Program

You have the opportunity to receive payroll credits of up to $650 by completing the activities listed in the table below:

Complete online well-being assessment* $50
Submit biometric screening results before November 30, 2018* $100
Obtain BMI of less than 30 or consult with a primary care physician* $250
Complete 2 preventive screenings ($50 each)
(For example: annual physical, well-woman's exam, dental exam, vision exam, etc.)
up to $100
Complete 3 designated well-being activities ($50 each) up to $150
Total Well-Being Incentive Program credit up to $650

* Note: These activities are required to be eligible for the Fitness Reimbursement

Fitness Reimbursement

You can receive a fitness reimbursement of up to $500, through payroll, which can be used for you and your eligible dependents enrolled in the medical plan. The total reimbursement of up to $500 can be requested once you have:

  • Completed a well-being assessment;
  • Submitted biometric screening results; and
  • Obtained a Body Mass Index (BMI) of less than 30 or have consulted with a primary care physician.

The fitness reimbursement includes most fitness related memberships and activities for example, CrossFit, yoga, Pilates, personal training, adult and child sports league fees, nutritional counseling and Weight Watchers program fees.

Biometric Screenings

A preventive biometric screening measures key health factors — from blood pressure and cholesterol to body mass and blood sugar — to help you identify risk factors for future health issues early. Phillips 66 covers the cost of this screening.

If you miss the onsite screening, you can also download a physician screening form from this site for your doctor to complete.
