Phillips 66 Company

Dependent Eligibility & Verification

At Phillips 66, we partner with employees to manage our health care costs, and one way to do this is by ensuring that only eligible dependents are enrolled in our plans. So before you hit the ground running, it’s important to verify that all of your enrolled dependents meet the required eligibility criteria.

Who Qualifies as a Dependent?

To see who qualifies as a dependent, view the documents below, as well as the summary plan descriptions listed under the Important Plan Information section on this page.

Dependent Reverification Project

Dependent Reverification Project

As part of the Phillips 66 Dependent Reverification process, some employees will need to provide documentation to verify the eligibility of their dependent(s) for coverage through the Phillips 66 Medical and Dental Assistance Plan. If the required documents are not provided, dependent(s) will no longer be covered by Phillips 66 benefits and will not be eligible for COBRA continuation of these benefits.
