As part of the Phillips 66 Dependent Reverification process, some employees will need to provide documentation to verify the eligibility of their dependent(s) for coverage through the Phillips 66 Medical and Dental Assistance Plan. If the required documents are not provided, dependent(s) will no longer be covered by Phillips 66 benefits and will not be eligible for COBRA continuation of these benefits.
