Power of Attorney

Phillips 66 will accept a power of attorney (POA) if you need to authorize an agent to make decisions or take an action concerning your benefit transactions*.

*To grant another person full or limited authority to act on your Phillips 66 Savings Plan account, or the authority to obtain information only, you must submit a separate standardized Vanguard Power of Attorney form to Vanguard.

Seeking a Medical Power of Attorney

Laws governing a medical power of attorney may vary from state to state. If you wish to establish this type of legal document for yourself, it’s important to work with an attorney.


To File a Power of Attorney (POA) with the Company

To file a POA with Phillips 66:

  1. Fax the POA to HR Connections at (918) 977-8003. If the notary seal is a pressed seal, please darken when faxing. Or you can mail your POA to:

  2. Phillips 66 must approve your POA before your authorized agent will be allowed to make a decision on your benefits. This process takes 2-3 weeks.
  3. Once the POA is approved, the named agent will receive a password from our Benefits Center, which is required when calling with benefit questions. If the POA is rejected, a letter will be sent to notify the agent.

  1. Obtain a Vanguard Power of Attorney form by calling Vanguard Participant Services at (800) 523-1188.
  2. Submit notarized form within 30 days to Vanguard in accordance with mailing information provided on the form.
